Dual Employment Form (Microsoft Word Document) if applicable
Social Security Exemption Form (Microsoft Word Document) if applicable
Continuation on Special Payroll:
- One (1) original payroll authorization
Offer letter
Rehire (break in service) on Special Payroll:
- One (1) original payroll authorization
Original or copy of original I-9 with Section 3 (Updating and Reverification) completed
if rehired within 3 years of the initial date of hire; or complete a new I-9 if rehire is more than 3
years
Offer letter
State and Federal w-4's needed only if individual wants to make a change to what was previously submitted
Dual Employment Form (Microsoft Word Document) if applicable
Social Security Exemption Form (Microsoft Word Document) if applicable
Change to previous authorization
- One (1) original payroll authorization
Revised offer letter needed for all changes except coding change
Gratis Authorization
- One (1) original payroll authorization

Special Payroll Employee Information Sheet
NOTE: All Special Payroll appointments must have a social security number in order to process
the authorization.
If the employee you are hiring on Special Payroll is currently on the Professional
or Graduate Assistant Payroll then the I-9 Form and Special Payroll Employee Information Sheet
will not needed at this time.
